This page contains application and notification forms that governing bodies must submit to the Non-State Schools Accreditation Board (NSSAB), and other templates to use for various functions.
Application forms
Submit applications to NSSAB on the forms below. We aim to make our decision within 6 months of receiving a valid, completed application which includes all relevant information and documents. However, sometimes it may take longer than expected. If this happens, we will advise you of a revised timeline. Please allow enough time for us to consider your application.

Change in governing body—CTGB-F | Apply to change a school's governing body from one incorporated entity to another |

Change of attributes of accreditation—CATT-F | Add additional attributes to an existing accreditation, such as years of schooling (e.g. Year 11 and Year 12), mode of delivery (e.g. distance education), gender, boarding, or special assistance. Note: The student-intake day / commencement day must be at least 6 months after the lodgement date |

Eligibility for government funding—EFGF-F | Apply for state government funding eligibility for an existing accredited non-state school |

New non-state school—NSCH-F | Use this form to apply for a new non-state school. Note: The student-intake day for the proposed school must be at least 6 months after the lodgement date |

New site and relocate site—ASIT-F | Apply for a new site for an existing school or to relocate an existing site. Note: The student-intake day must be at least 6 months after the lodgement date |

New type of education—NTE-F | Add primary or secondary education if the school is accredited for only 1 of these types. Note: The student-intake day must be at least 6 months after the lodgement date |

Submit notifications to NSSAB on the forms below and include all relevant information and documents. If needed, we may ask for more information or documents to ensure all requirements are met.

Amalgamation notice—AMAL-F | Notice to inform us of the intention of 1 or more governing bodies to merge 2 or more schools into 1 accredited school. Note: The proposed governing body of the amalgamated school must submit this notice. You cannot lodge this notice if a school's governing body has an unresolved compliance, show cause or information notice |

Attribute of accreditation no longer applying—ANLA-F | Inform us that an accreditation attribute no longer applies to the school, such as the site, mode of delivery, year levels, gender, boarding, or special assistance |

Change in circumstances—CCIR-F | Use this notice to inform us of a change in circumstances. Note: Notification timeframes apply. The school's governing body must provide notice within 7 days or 28 days of certain events occurring |

Division notice—DIVN-F | Advise us of a governing body's intention to divide an existing accredited school into 2 or more separate schools. Note: You cannot lodge this notice if the school's governing body has an unresolved compliance, show cause or information notice |

Name change notification | Notify us of a name change for a school or governing body. Note: Updates to principal and school details can be made through
NSSAB online services |

Special assistance school (SAS)—intention to use temporary special assistance site—TSAS-F1 | Use to inform us of the intention of a governing body of a special assistance school to provide, on a temporary basis, special assistance at a temporary site. Note: Temporary special assistance sites cannot be used for more than 200 consecutive school days |

SAS—intention to stop use of temporary special assistance site—TSAS-F2 | Notify us that the governing body of a non-state special assistance school has stopped providing special assistance at a temporary site. Note: Temporary special assistance sites cannot be used for more than 200 consecutive school days unless you lodge a site inclusion application. This will allow you to continue using the temporary site for a specified period |

Surrender notice—SURR-F | Update us of the surrender of a type of education for a non-state school. Note: You will need to lodge this notice before the date the surrender takes effect |

Downloading documents
To download a form, notice or template, select either the corresponding reference number or name. Once the file is open, make sure it is saved to a location on your local drive.
Alternatively, to download a document:
- Right-click on the relevant ref. no or form name.
- Select 'Save target as...' or 'Save link as...'
- Save it to a location on your local drive.
Submitting documents
Lodge your form or notification electronically, preferably through
NSSAB online services, or if this is not an option for you, via email at
nssab.admin@qed.qld.gov.au.
If you cannot lodge it electronically, mail a printout of the completed document to us. Find our postal address.
